At WestMass ElderCare, we participate as a key member of the Senior Care Options team, a comprehensive health insurance plan that covers all the services typically paid for through Medicare and MassHealth. SCO integrates healthcare with social support services by coordinating care and in-home support services through a collaborative Care Team for members age 65 or older.
WMEC works closely with several health insurance providers, including Commonwealth Care Alliance, Fallon Navicare, Senior Whole Health, Tufts Health Plan, United Health Care, and WellSense Health Plan (formerly BMC HealthNet Plan). These insurance companies SCO plans to provide a range of services and benefits to eligible individuals.
Within the Care Team, WMEC’s Geriatric Support Services Coordinators (GSSCs) provide assessment and coordination of in-home and community services, working closely with members, their families and caregivers, Primary Care Providers, Specialists, Nurses (Care Partners), and Behavioral Health providers. The GSSCs play an essential role in developing personalized care plans tailored to each member’s specific needs and preferences.
We also work with individual aged 21-64 who are enrolled in One Care plans which serve persons who are enrolled in MassHealth and Medicare. Services such as Adult Family Care, Personal Care Assistance, and Home-Delivered Meals programs can be part of a person’s care plan for those who are enrolled in a n SCO program.
