WMEC Care Managers and RNs support older adults and persons with disabilities and their caregivers by developing care plans and helping to access much-needed resources. Using a person-centered approach, we create a unique plan customized for you based on your needs and wants.
Adult Family Care (AFC) connects individuals 16 or older who need assistance with daily living activities but desire living in a family setting with community and family members willing open their home. Hosts can get paid for providing the room, transportation, meals and care they often already are.
Our Personal Care Attendant (PCA) programs allows you to choose family or friends to provide your care. Skilled PCAs help older adults and people with disabilities or chronic disabling with regular daily tasks, such as dressing, bathing, laundry, medication management, cooking, and more.
WMEC on-site Resident Services Coordinators (RSCs) support residents of designated housing sites in Holyoke, South Hadley, Chicopee, and Ware. RSCs arrange and coordinate services and organize resident activities.
Program options can be overwhelming. Our Options Counseling service provides in-home, free guidance to help you navigate choices and challenges you may face based on your needs and desires.
Whether you are the caregiver of an older adult or person with dementia, or a relative raising nieces, nephews or grandchildren, you deserve support yourself and to know you are not alone! Our Family Caregiver Support services are here to help.
