WestMass ElderCare partners with health care plan providers as part of our consumers’ primary care teams. This allows us to offer care and service coordination for seniors and people with disabilities, ensuring members of the Western Mass. community are able to stay independent, empowered, and safe.
Whether you’re a member of a Senior Care Option Organization (SCO), One Care (OC) or Long-Term Services and Support Community Partner (Care Alliance of Western Massachusetts), our staff work to make sure that you are supported with community, care-at-home, and care transition resources.
Senior Care Options (SCO) is a comprehensive health insurance plan that covers all of the services normally paid for through Medicare and MassHealth. SCO integrates health care with social support services by coordinating care and in-home support services through a collaborative Care Team which includes older adults, family and caregivers, Primary Care Providers, Specialists, Nurses (Care Partners), Geriatric Support Services Coordinators (GSSCs), and Behavioral Health providers.
As key members of your Care Team, WMEC’s GSSCs provide assessment and coordination of in-home and community services. Meet our SCO Care Teams:
WMEC’s Integrated Health Plans provide case management support, coordinate home- and community-based care, and act as the main link to primary care teams for:
One Care is a comprehensive health insurance plan that covers health care services along with a wide range of in-home and community support services for adults under age 65 who have both MassHealth and Medicare. WMEC’s Long-Term Support Services Coordinators (LTSC) work with both of the One Care plans as members of the person-centered care teams. LTSCs provide service coordination for One Care members with in-home supports and help to navigate community resources and referrals.

Long-Term Services and Support (LTSS) Community Partners (CP) are community-based entities that work collaboratively with Accountable Care Organizations (ACOs) and Primary Care teams to provide care management and coordination for MassHealth ACO members with complex LTSS needs.
