Who We Serve:

Individuals over the age of 60 years old who need help remaining independent in their own homes.
Individuals under the age of 60 years old with Alzheimer’s disease or dementia.
Individuals who need assistance with day-to-day activities such as shopping, laundry, light housework, and meals.
Individuals with several medical conditions who would benefit from medication reminders.
Individuals who are concerned about falling or have a history of falls. In this case, a Personal Emergency Response System can be provided to keep you safe.

What is the cost for Home Care services?

The cost for services is based upon an individual’s annual income. The cost for individuals with lower income is minimal while others may pay a percentage of the total cost for services. There is no charge for an in-home assessment and ongoing Case Management support.

How Can I Access the Home Care Program?

A Care Manager will conduct a comprehensive assessment of your needs to determine your eligibility. Eligible individuals generally need assistance in multiple areas of their daily lives. If you are not eligible, we can connect you to other options that meet your care needs.

My daughter/son recommended that I receive help with meals. Do I have to accept them?

Your service plan is determined based upon your needs and wishes. You decide what services will be a part of your service plan.
